Brief description of the problem (reason for recall)
Adcock Ingram Critical Care (AICC) is issuing a Medical Device Recall as the distributor for certain lots of HomeChoice Sets with 4-prong Cassettes due to an issue associated with an increase of reports of alarms while using the HomeChoice cycler. The most frequent alarms, “Check Lines & Bags” and “Reload the Set 156”, may occur and repeated alarms could prevent the HomeChoice system from initiating the therapy cycle.
Advice for health professionals and distributors:
The manufacturer, Vantive, has determined that this issue is caused only by specific cassettes from lots listed in the affected product table below: This issue could lead to a delay or interruption of therapy. In addition, it may potentially lead to touch contamination of the sterile fluid path due to failure of following aseptic technique as stipulated in the Patient Guide as a result of having to repeatedly change sets.
